Leonard passed away on March 5th at the Thunder Bay Regional Hospital at the age of 71.

He is survived by his brother Larry Chudak, sister Gloria Chudak, and niece Jennifer Chudak.

The family would like to thank the staff of Red Lake Cochenour Memorial Hospital, Thunder Bay Regional Hospital, and St. Joseph's Care Group. A special thank you to Susan and Reggie Molloy, Larry Toderian, John Clement and family, and JP Bouchard.

Sadly missed and forever remembered.

As per Leonard's wishes a private interment will take place at a later date.
